EFAD and the ESDN Primary Care will contribute to the programme through a workshop entitled “From Invisible Data to Essential Care: Redefining the Dietitian’s Impact in Multidisciplinary Primary Care Teams,” led by Isabel Monteiro, ESDN Primary Care Lead and Wineke Remijnse, EFAD Ex. Director. In addition, Chiara Perrone, ESDN Primary Care member, will participate in a poster debate exploring the complexity of intersectoral cooperation from hospital discharge to primary care.

The conference provides an important opportunity to showcase the contribution of dietitians to integrated, person-centred primary care across Europe.
